Health Plans and Employers
Employers and Health Plans can reduce total cost of care for their high risk members by reducing readmissions and hospitalizations. Telehealth programs can even help to reduce visits to Emergency Room. The programs can be tailored for managing high risk members with CHF, COPD, Diabetes, ESRD, and other disease states.
Some Payors (Blue Cross Blue Shield) who are more experienced with disease management and patient monitoring than others, after careful evaluation, including extensive testing of our infrastructure, selected Carematix to help manage their high risk members. Carematix has experience serving Commercial, Medicare, Medicaid and Special Needs Programs in all US states.
Carematix wireless devices are placed in patients' homes to take their vital signs every day for review by their nurses from any computer anywhere. Patients transmit their blood pressure, oximetry levels, pulse rate, weight readings daily with equipment installed in their home. If any reading is outside the parameters set by their nurse, a notification is sent to the nurse. This realtime feedback helps healthcare professionals detect and address even the slightest abnormalities before they become serious, preventing emergency room visits and re-hospitalizations. Followup can also be done via Automated Phone calls which can be used to provide medication reminders, collect responses etc.
This low cost, easy to use system is the favorite among payors. Since everything is web based and wireless, it is very simple to setup and use.
Who should get Telehealth?
Candidates to be considered are patients with:
Carematix solution is modular in design and can be customized per patient according to physician care plan directive. A number of vital signs can be taken from a patient including: blood pressure, oxygen saturation, pulse rate, weight (up to 400 lbs.), peak flow and blood glucose. Given new rules from FDA, we may be able to work with you on specific programs for monitoring Creatine levels, CRP levels etc.
A physician can choose to have a patient’s vital signs monitored multiple times throughout the day as needed. Clinical questionnaire assessments and medication reminders can be customized and delivered over automated phone calls.
Carematix technology brings benefits to both patients and health care professionals. When patient’s vital signs are taken on a daily basis, irregularities can be caught before they become full-fledged problems. Nurses can be sent to patients’ homes at unscheduled times in addition to regular nursing visits, and based on the daily data, work directly with the patient’s doctor to adjust medication and diet regimens. Enrolling a patient in a telehealth program has consistently shown:
In one study, airline pilots participated in a study monitoring weight and blood pressure readings. The FAA requires pilots to maintain healthy vital signs to be eligible to fly, so for the pilots who volunteered for this study their livelihood was directly related to their health!! This study sought to find a link between consistent monitoring and a healthier lifestyle. During this study pilots were encouraged to take readings on both the scale and blood pressure monitor multiple times per week. Over the course of the study, many of the participants saw a greater than 10% reduction is either systolic or diastolic blood pressure. Many of the participants reported high levels of satisfaction, ease of use and willingness to endorse the Carematix system to fellow pilots.
